Provides advocacy, coordination and continuity of evidence-based care for individuals and families in need through a collaborative process of triage, assessment, planning and communication. Participates as a member of a multidisciplinary treatment team. Reports to the unit/department supervisor.
Child welfare professional with at least two years of experience working with at risk population to include families experiencing substance disorder, domestic violence and mental health problems. Child welfare certification preferred. Bachelor degree in Human Services or related field. Spanish speaking preferred.
